Patient Rights & Compliance Information

In accordance with Texas House Bill 4224 (Texas Health & Safety Code §181.105), Wellness First Medical Center provides the following information to all patients.

How to Request Your Medical Records

You have the right to access and obtain a copy of your medical records. To submit a request:

  1. Contact our front desk in person at 350 FM 517 Rd. W., Dickinson, TX 77539, or by phone at 281-337-3337.
  2. Submit your request in writing. You may be asked to complete a Medical Records Release Authorization form.
  3. Include your full name, date of birth, dates of service, and a description of the records you are requesting.
  4. Specify whether you’d like records sent to you directly, transferred to another provider, or picked up in person.
  5. Requests are typically processed within 15 business days. A reasonable fee may apply for copying and handling.

How to Contact the Licensing & Disciplinary Authority

  1. Visit the Board’s website and navigate to the “File a Complaint” section.
  2. Complete the online complaint form or download and mail a written complaint.
  3. Include the provider’s full name, license number (if known), a description of your concern, and any supporting documentation.
  4. The Board will review your complaint and contact you regarding next steps.

How to File a Consumer Complaint

Under Texas Health & Safety Code §181.103, patients may file a complaint regarding the handling of protected health information (PHI). To file a complaint:

  1. Visit the Attorney General’s website and select “File a Complaint.”
  2. Choose the appropriate complaint category related to healthcare privacy or consumer protection.
  3. Complete the online form with your contact information and a detailed description of the issue.
  4. Submit any supporting documentation, such as correspondence or records related to your complaint.
  5. You will receive a confirmation and the AG’s office will follow up as appropriate.
